A DAY IN THE LIFE AS A LPNAC (Licensed Practical Nurse Assessment Coordinator)
You work with an experienced team and knowledgeable outside support to prepare assessments for residents at our 324-bed Skilled Nursing Facility. Your goal is to ensure that the services they receive are appropriate and effective as we strive to provide the highest quality of care possible. In addition to coordinating and conducting full comprehensive MDS assessments and quarterly reviews for our residents, you will also play a pivotal role in our medical care evaluation studies for Quality Assurance. Your attention to detail and clinical experience enable you to evaluate identified changes in condition and initiate subsequent actions. Your dedication to quality care and your interpersonal skills help you as you not only review the residents’ medical and psychosocial history but also conduct interviews with the resident, families and Cedar Haven team. WORK DETAILS

QUALIFICATIONS FOR A LPNAC (Licensed Practical Nurse Assessment Coordinator)
• Current Pennsylvania LPN license and graduation from an accredited school of professional nursing
• A minimum of 2-3 years of nursing experience
• LPNAC experience strongly preferred
• Current LPNAC Certification strongly preferred or willingness to obtain certification
