What You’ll Do:

As a Home Health/Hospice CNA: Certified Nursing Assistant, you will carry out our Mission of Love by performing duties as outlined in the plan of care essentially pertaining to personal care and socialization which would be given to the terminally ill persons in their homes. Helps the family members and caregivers to carry on their share of the responsibilities and encourages the patient to participate in the life of the family in so far as able.

Our Home Health/Hospice CNA: Certified Nursing Assistant is responsible for the following:

Attend trainings to build understanding and capacity to undertake QAPI work. Carry out QAPI roles and responsibilities as assigned.
Takes and records accurate vital signs including patient weights and arm circumferences.
Describes the activities of daily living and demonstrates the ability to use techniques to assist the patient to meet these activities until no longer able.
Positions patient with proper body alignment.
Assist with keeping patient care area neat, clean and tidy. Replenish personal care items as needed.

What You Have:

3-5 years of experience in Hospice is required.
Must have a current and valid Nursing Assistant certification.
A high school diploma or GED is preferred.
